About
Dr. Nirali Shah Jain passed a surgical examination before she became a fertility specialist. Fundamentals of Laparoscopic Surgery is an objective test of laparoscopic technique — suturing, knot tying and task execution on a box trainer — and she earned certification in 2020, the same year the Society of Laparoscopic & Robotic Surgeons named her its Outstanding Minimally Invasive Surgery Resident. She is a reproductive endocrinologist who trained as a surgeon first, in the residency years, and that ordering shows up in how she evaluates whether pelvic pathology should be handled before a cycle.
Her training ran through three cities. Northwestern University for neurobiology with a minor in dance, then the same university’s Feinberg School of Medicine for the MD. Obstetrics and gynecology residency at New York Presbyterian / Weill Cornell, where she served as co-Chief Resident in her fourth year (2020-2021) — the most responsibility a residency hands out, and typically given to whoever will be trusted with the sickest and most urgent cases. Reproductive endocrinology and infertility fellowship at NYU Langone Medical Center.
She practices at two RMA offices, Basking Ridge and Springfield, and describes her own approach as refusing the menu model: “Infertility medicine is more than just offering standardized treatment options — it involves learning each person’s unique story and understanding their goals, standing by their side through the highs and lows that the IVF process inevitably brings.”
Clinical Expertise
- Reproductive endocrinology and infertility: workup, stimulation design and ART cycle management as an RMA treating specialist
- Third-party reproduction: her primary research interest — donor eggs, donor embryos and gestational carrier arrangements, each with a different medical sequence, legal layer and disclosure decision
- Oncofertility: fertility preservation for patients about to start chemotherapy, radiation or pelvic surgery, where the retrieval has to be scheduled around someone else’s treatment calendar
- Laparoscopic and minimally invasive surgery: FLS-certified and awarded for operative performance in residency, which covers pelvic and uterine pathology that affects conception
- Ovulation and cycle assessment: she has written a full guide to ovulation, and clinically this is monitoring, ovulatory dysfunction, and deciding when a cycle is not behaving as the protocol assumes
- Access and cost in US fertility care: she has published an analysis of the barriers to fertility care in America — insurance coverage, cost, and state-by-state variation as clinical constraints rather than afterthoughts
Why Choose Dr. Nirali Shah Jain
She holds an operative skills certification, not just a surgical interest. FLS is an examined credential rather than a course attended, and the SLS Outstanding MIS Resident Award was given by a surgical society for operative performance. For a patient told they may need fibroid or endometriosis surgery before IVF, a physician who can judge the operative question herself can answer “operate first or retrieve first” without a referral round-trip.
Co-Chief Resident is a role assigned under pressure. The chief resident year at a large urban hospital means carrying the hardest admissions and the busiest call. Doing it at New York Presbyterian / Weill Cornell is a training-program judgment about independent decision-making, and it precedes her subspecialty fellowship.
Her research populations cannot wait. Cancer patients need preservation completed before gonadotoxic treatment starts, sometimes inside a week. Third-party reproduction requires matching, legal steps and psychological preparation before the medicine can proceed. Both are named research interests, which means she has worked the questions rather than only the cases.
Two offices, one physician. Stimulation monitoring means frequent visits, sometimes daily near trigger. Practicing in both Basking Ridge and Springfield lets a patient in central New Jersey monitor close to home without switching doctors mid-cycle.
She is an ASRM-sphere scholarship recipient. The 2023 Pacific Coast Reproductive Society Scholarship is a specialty-society recognition. Her earlier awards were surgical; this one is in the reproductive field she moved into.
She says the appointment is not complete at the plan. “My work is not complete until my patients achieve their goals,” is her framing, and she has talked publicly about helping friends and family through their own infertility — which tends to make a physician fluent in what the process feels like from the side without the stethoscope.

Awards include the Society of Laparoscopic & Robotic Surgeons Outstanding MIS Resident Award (2020) and the Pacific Coast Reproductive Society Scholarship (2023).
Suitable For
- Donor egg, donor embryo or gestational carrier paths: her main research direction, and a population whose hardest decisions are often non-medical — matching, parentage, and how a child learns their origin
- Preserving fertility before cancer treatment: when the window is set by an oncology start date, the scheduling and stimulation approach matter more than the average patient realizes
- Fibroids, endometriosis or adhesions possibly needing laparoscopy: she is FLS-certified and can weigh treating the pathology against the cost in cycle time
- Central New Jersey patients who need frequent monitoring: two RMA offices, so bloodwork and ultrasound can happen near home without changing physicians mid-treatment
- Patients new to treatment who want each step explained: her stated approach is individualized rather than a menu, and she writes about access and cost barriers — useful if you want the reasoning and the bill discussed in the same visit
