About Florida Institute for Reproductive Medicine
Founded in 1990, Florida Institute for Reproductive Medicine (FIRM) is one of the most trusted fertility centers in Jacksonville, Florida, and the broader North Florida and South Georgia region. With over 35 years of clinical experience and celebrating its 35th anniversary, the center’s own site states that more than 20,000 babies have been born to its patients. The practice calls itself the largest provider of infertility care in North Florida and South Georgia — its own claim, not a registry finding.
FIRM follows the “3 C’s” philosophy — Care, Cost, and Convenience — led by board-certified Reproductive Endocrinology & Infertility (REI) specialists:
- Kevin Winslow, MD — Founder and Medical Director; he has directed the practice for over 21 years and brings more than 36 years of clinical experience, with a special interest in IVF and fertility preservation
- Teresa Erb, MD — ASRM member, ACOG Fellow, board-certified in OB/GYN and REI
- Adrienne Gentry, DO — Board-certified in OB/GYN and REI, ASRM member
- Viji Sundaram, MD — ASRM member, ACOG Fellow, board-certified in OB/GYN and REI
The main downtown Jacksonville facility houses both the embryology and andrology laboratories, staffed by three doctoral-level and one master’s-level embryologists; the practice reports that its infertility nurses average more than 8 years of experience, with several past 15 years at FIRM.
FIRM publishes no results page of its own; its outcomes reach the public through federal reporting. The CDC’s 2022 ART record for this clinic shows 1,114 cycles in the reporting year and live-birth rates of 37.2% under 35, 34.5% at 35–37, 17.7% at 38–40 and 6.2% over 40. That CDC page carries a data-verified attestation from the practice’s medical director, Kevin L. Winslow — provenance most clinic-published figures do not offer. Two caveats keep the numbers usable. The federal record for this clinic reports no fresh-embryo versus frozen-transfer split, so those four bands are aggregate per-age figures rather than a transfer-type table, and the capture does not name the denominator behind each band; ask the clinic whether any figure you are given counts intended retrievals or transfers. The national row in the chart below is SART’s 2022 national summary (53.5 / 39.8 / 25.6 / 9.5), a different registry with its own reporting conventions, so the two rows are not a like-for-like comparison — the steady decline with age is the pattern that holds in both. FIRM’s site displays a SART badge and footer branding; our sources record no SART ClinicPKID for this clinic, so we report that badge as the clinic’s own display rather than registry-confirmed membership.
Accreditations & Awards
- Newsweek, America’s Best Fertility Clinics 2025 — announced on the clinic’s own news page. A 2026 Newsweek rank of 53 sits in our data notes as user-supplied rather than captured from Newsweek, so we do not print it as a finding.
- SART badge on the clinic’s site — clinic-displayed. Our sources record no SART ClinicPKID for FIRM, so we do not print that badge as registry-confirmed membership.
- CDC ART reporting — FIRM appears in federal assisted-reproduction reporting, which is where its cycle data can be checked rather than on a clinic results page.
- No CLIA certificate number is recorded in our sources for this clinic’s laboratory, although the main facility documents on-site embryology and andrology labs. Ask to see the number.
- Media coverage — the program has been featured on NBC, CBS, BBC News and the Today Show, and in People, Self, Pink, Conceive, Child and Woman magazines. Coverage is not accreditation.

Fees & Packages
There is no published price list for a cycle at this clinic. What FIRM does put in writing is how money is arranged, and it is the clinic’s own reporting in every case below — not a third-party benchmark.
- A below-average fee claim: the practice states that its fees “average about 10–20% below national averages,” framing cost as one of its three C’s.
- 10% discount on eligible services for active military personnel, police officers, firefighters and teachers. The clinic asks that you raise it at your IVF consultation.
- Cost-sharing and guarantee IVF programs, described as aimed at patients with lower chances of success, plus financing options across infertility care with LendingClub named as a lender.
- Insurance: FIRM says it accepts all major insurance policies, including all forms of Tricare and Tricare Prime, and names Blue Cross Blue Shield, Aetna, Cigna, Progyny, Humana and United Healthcare among the carriers it works with. Its own guidance is that most insurers cover infertility diagnostics and most do not cover treatment, so verify your policy — the billing department will help — before a cycle starts.
- An in-house pharmacy, presented by the clinic as part of holding total cost down.
- No state mandate to fall back on: only 24 U.S. states require infertility coverage, and Florida and Georgia are not among them. Only a minority of large employers offer it either, so budget from your own policy rather than from a general rule.
Ask, in writing, which items sit outside a quoted package: monitoring visits, medications, anesthesia, ICSI, PGT biopsy, freezing and annual storage.
Patient Journey
- Initial Consultation — Meet with a physician to review medical history and develop a personalized treatment plan
- Fertility Evaluation — Hormonal testing, semen analysis, ovarian reserve assessment
- Treatment Planning — Physician recommends the most suitable treatment based on evaluation results
- Ovarian Stimulation & Monitoring — Medication to stimulate follicles with regular ultrasound monitoring

Stimulated cycles mean frequent morning visits. Every office the clinic documents keeps 7:00 am to 4:00 pm, Monday through Friday, closing for lunch from 12:00 pm to 1:00 pm each day, and calls placed to the Daytona Beach office after hours route to the answering service in Jacksonville. Our sources document no weekend or early-monitoring schedule for this practice.
Local Legal Framework
Florida law permits egg donation and embryo donation. Surrogacy is restricted in Florida — commercial surrogacy is not permitted. PGT-A and PGT-M genetic screening are legal in the United States. LGBTQ+ individuals have equal access to fertility treatment at this practice. Our sources for this clinic state these points without naming a Florida statute, so we do not supply citation numbers for them; treat each as a starting point for a conversation with an independent Florida family-law attorney rather than as advice, and have any donor or carrier agreement reviewed before you sign.
